Decoding Macra For Emergency Physicians

It has been a hot topic for months, and rightfully so. MACRA, or the Medicare Access & CHIP Reauthorization Act (also referred to as the “SGR-fix”) was created to drive physicians towards the ultimate trifecta of better care, smarter spending, and healthier patients. Basically, it will reward healthcare providers for giving better care instead of more service. But what does it all mean for Emergency Physicians?

DECODING TARIFF IMPACT

Tariffs are a major force shaping supply chain costs and procurement decisions in 2025. With new rules under Section 232 and Section 301 of U.S. trade law, the International Emergency Economic Powers Act (IEEPA), and reciprocal tariffs, the rates are no longer static. They vary by country, spend category, and political climate. Enterprises are dealing with supplier price increase requests, unclear cost deltas, and confusion over applicable tariff rates and their duration, all while managing rising compliance risks and audit requirements. It’s critical for companies to understand how tariffs affect landed costs, when they can push back on supplier price increases, and when they need to rethink sourcing altogether

PROCUREMENT IS NAVIGATING A TECH TSUNAMI

Procurement is right in the eye of a tech storm. The economic currents have shifted, and familiar navigation tools are no longer enough. Traditional models are under pressure — from faster decision cycles, growing complexities of global trade driven by tariffs, and rising expectations for intelligence and integration. This isn’t routine turbulence or a rogue trend wave; it’s a structural reset that is reshaping how procurement operates, delivers value and stays aligned with the business. Technologies like agentic AI aren’t just incremental upgrades; they represent a generational leap — systems that can learn, decide and act with minimal oversight. And yet, for all their promise, these advancements come with complexity. The road ahead is rich with opportunity but riddled with pitfalls. Successfully navigating this terrain requires more than enthusiasm. It demands fluency.

Your Guide to Substance Use Management

Why substance scope matters 01. SUBSTANCE SCOPE Substance use doesn’t just create direct treatment costs, it quietly drives up spending across cancer, heart disease, mental health, and other chronic conditions. Individuals with both an SUD and a chronic illness have twice the healthcare costs of those with a chronic condition alone.3 Substances like tobacco, alcohol, and cannabis are often treated as secondary concerns but they’re among the most preventable and costly drivers of long-term health claims. According to a 2025 U.S. Surgeon General advisory, alcohol and tobacco are two of the top three leading preventable causes of cancer, with alcohol alone linked to seven different types.4

AI’s Achilles’ Heel The Supply Chain Crisis Holding Back the Future of AI

Artificial intelligence is accelerating at a pace few technologies have ever seen. But behind every leap in capability lies a more complex reality — one built not on code, but on concrete, copper, silicon, and power. Companies are racing to construct new data centers, deploy faster networks, and secure the electricity needed to sustain AI at scale. Yet, the infrastructure that makes all this possible is under growing pressure. The global supply chains that support it are fraying. Delays, shortages, and rising costs are becoming the norm. Without a coordinated response, the physical limits of these supply chains will soon become the limits of AI itself.

What’s Next After PBM Reform?

In 2024, the U.S. Congress set the stage for a monumental shift within the pharmacy care industry by introducing nine federal legislative initiatives targeting Pharmacy Benefit Management (PBM) companies1 . Although none were enacted into law last year, the bipartisan effort to reform the PBM industry continues into 2025. These initiatives promise to fundamentally alter the business models and operational dynamics of traditional PBMs. [1]: Mintz.com https://www.mintz.com/insights-center Against this backdrop of anticipated PBM reform, EmpiRx Health has pioneered a paradigm shift away from the volume-driven business model of the big, traditional PBMs towards a pharmacist-led solution that prioritizes patient health over self-dealing profit generation. EmpiRx Health’s clinically-driven pharmacy care model is virtually unique in the PBM world. By centering clinical pharmacists in the PBM service ecosystem and prioritizing clinical outcomes over drug sales and rebate incentives, the EmpiRx Health approach is not just compliant with the proposed PBM reform legislation, it sets a new standard for what pharmacy benefit management can and should be.

24-hour posture and pressure management for Occupational Therapists

This guide reminds us that posture is inescapable. Whether we are sitting, lying, standing, or walking, posture is inextricably linked to other condsiderations. Elements such as mobility, pressure care, muscle functioning, spasticity, kyphosis, scoliosis, and more are vitally connected with posture. Which, in turn, relates to associated equipment needs including that of beds, chairs, seating support systems, and wheelchairs. The bread and butter of occupational therapy. I come to the guide as a non-clinician with an interest in law. What catches the eye is the emphasis on clinical reasoning, crucial both professionally and legally. It may be the basis for arguing for high-cost items, to meet need, even in the teeth of limited resources. Conversely, it is also the basis for defending a decision not to provide something. For example, some recent legal and ombudsman cases, involving decisions about equipment and care provision, have focused on the presence (or lack) of plausible evidence and reasoning. And, of course, professional registration standards also demand adequate recording of decision-making.

Digital breathlessness pathway with Queen Victoria Hospital NHS Foundation Trust

Breathlessness is associated with high healthcare use, accounting for 5% of presentations to emergency departments1 2, approximately 4% of GP consultations3 and reported by patients in 12% of medical admissions4. People present with breathlessness in primary care and the causes can be numerous. Due to this complexity it can result in a delayed/inaccurate diagnosis, people not getting the treatment/care they need, and unnecessary hospital admission. As there is rarely a single cause of breathlessness, multiple investigations are likely to be required, and a holistic approach needed. Early and accurate diagnosis is critical to ensure the best outcome for the patient.

AIOps is not observability AIOps drives value for observability.

The adoption of modern software delivery and distributed systems created the need to ingest and manage vast amounts of data through observability. As the adoption of observability grows, IT must deal with an unprecedented amount of data. Every application, infrastructure, and network across complex hybrid environments produces digital exhaust, generating data at overwhelming speeds and in different formats. This raises the question: how do IT teams manage all this data? IT teams need to improve and enable business operations. Is observability enough? And not only to make sense of all of it, but also connect it with the broader business operations.

Assessment of quality and total cost of care across individual doctors: Benefits and methodology

Why assess individual doctor performance? The U.S. healthcare system generates significant economic waste at nearly $1 trillion per year while its quality outcomes are currently 37th on the World Health Organization's rankings. Garner has spent years analyzing data to better understand the causes of poor outcomes and what can be done to improve the results. We found that individual physician behavior is the most important factor in determining the cost and quality of an episode of care. Our data shows that in every geography in the country there are significant differences in how doctors practice. These differences create dramatically different health outcomes for patients and different costs for employers and plan sponsors. As an example of the overall impact of doctor selection, our data shows that the top-performing doctors in every geography add three healthy days to their patients' lives each year while simultaneously lowering the total cost of care by 32%.

The journey to passwordless for healthcare

While all industries contend with cybersecurity threats, healthcare is among the most-targeted industries, due in large part to the criticality of patient care, as well as the perceived value of PHI. Bad actors attempt to gain access to systems in many ways, but passwords are a common target – and the weakest link. Healthcare CIOs and CISOs seek to eliminate or mask passwords from end user workflows and systems. However, they currently struggle with the absence of a viable solution that both keeps systems secure and enables end users.

Performance and Applications of Helix’s Exome+ Assay

Helix’s Exome+ ® assay is run on all samples sequenced at Helix. This single assay combines a clinical exome with uniform panel-grade tests, a genome-wide backbone, and full mitochondrial coverage. Post sequencing, rigorous quality checks are applied to ensure that the data generated are appropriate for a broad application of clinical genetic tests. These tests range from population health screens, to diagnostic or pharmacogenomic panels to whole exome sequencing. Meanwhile, the Exome+ backbone of whole genome single nucleotide polymorphisms (SNPs), augmented with imputation, enables the generation of ancestry, traits, and polygenic risk scores. By ensuring that this single assay is appropriate for answering a wide list of clinical questions, the Exome+ assay establishes a foundation for Sequence Once, Query Often ® . Sequence Once, Query Often is a process that allows various genetic test results to be delivered for an individual over the course of their life based on when each might be relevant or informative. In building a test menu relying on the Exome+ assay, we have focused on three important pillars: quality, breadth of testing, and turnaround time, as discussed in the following sections.

Generative Artificial Intelligence Applications to Extract and Structure Clinical Trial Data from Unstructured Data Sources

Modern clinical trials have become increasingly sophisticated in design, as well as the type, volume, and variety of data collected for precise protocols. The complexity of clinical trial protocols is due primarily to the targeted nature of new drugs and a detailed stratification of patients based on inclusion/ exclusion criteria. Research subject profiles are more granular and trial endpoints are more nuanced, requiring additional data on molecular, genetic, and other types of biologic information. A recent report by the Tufts Center for the Study of Drug Development identified that a phase III clinical trial generates 3.6 million data points on average – which, for scale, represents three times the total number of data elements collected during late-stage trials just ten years ago.1

Clinical Chemistry Analyzer Market on the Rise: What’s Driving Demand?

The core technology behind clinical chemistry analyzers isn’t changing, said Stephen Harding, PhD, vice president of research and development at The Binding Site, part of Thermo Fisher Scientific. Instead, expect AI to have a growing influence on the results that analyzers produce. “If we start thinking about where we will go in the next three to four years, the world is going to be dominated by higher sensitivity systems, such as mass spectrometry and nextgeneration flow cytometry and cell staining,” Harding said. “The higher the sensitivity, the more reliant it will be on AI solutions to interpret.” “Whilst clinical chemistry analyzers will be faster with higher throughput and use the same technologies … the revolution that’s coming will be around sensitivity and AI interpretation,” he added.
